Care Coordinators

Care Coordinators are part of a GP practice’s multidisciplinary team, working alongside GPs, Pharmacists, Nurses, Social Prescribing Link Workers, Health and Wellbeing Coaches, and other appropriate professionals.

They help manage a patient’s care by coordinating an all-encompassing approach to personalised care by offering them extra support to help manage their health condition.

The role of a Care Coordinator:

  • We work alongside GPs to give extra support to patients, helping them create a personalised care and support plan based on what matters to them
  • We connect patients to multiple appropriate services that can benefit them
  • We provide coordination of care and support across healthcare services for patients

Appointments and Referrals

Care Coordinators will work with GP practices to identify patients in need of their support. External referrals are not received directly into the service.

GRAND UNION – If you feel that the service may help your patient, please make a referral to our Care Coordinator using the ‘Care Coordinator Referral Form’ in SystmOne. After completing the referral form, please click on ‘Task’ to assign it to our team.

Locations

Care Coordinators employed by GPA are currently located in Grand Union and ARC PCNs.

Frequently Asked Questions

Care Navigation – The service provides care navigation for patients aged 18+ who need assistance in connecting with financial, practical, emotional, and social support services.

Cancer Care Reviews – The service provides cancer care reviews based on QOF criteria and offers additional support to cancer patients.

Frequent Attenders – The service provides support for patients who are frequent users of the GP practice, if they are 18+, with a long-term health condition, and have attended at least 25+ GP appointments in a year.

Group Clinic Consultation – Group clinics offer supportive peer group settings for individual consultations for depression, menopause, and chronic pain.

Safeguarding Support– We attend safeguarding meetings and provide support to patients 18+ who need linking to our personalised care teams and other services.
