We’re seeking a dynamic Psychosocial Practitioner to join our Collaborative Care Team on a full-time, fixed-term 12-month contract. This is a fantastic opportunity for an entry-level Occupational Therapist or Social Worker to make a real impact by empowering others and improving care for those with long-term conditions.
In this role, you’ll assess referrals, create personalised care plans, and help service users manage their health and independence. You’ll work closely with GPs, social care teams, hospitals, and community organisations to ensure seamless support and tackle health inequalities.
